Having visited many many Piazza's both in Italy and across Central Europe can say that this is a big one... it's pretty and during our visit in Feb 17 it wasn't that busy, But I can imagine in the height of summer it will be chockablock.

This area is surrounded by police all the time as its Milan top attraction, but none of the police seem bothered about the hundreds of vendors that try and tie a bit of cotton bracelet on any part of your body and charge you for it. I have visited Duomo couple of times; when it was free to visit and climb to the roof and when it was more restricted and paid visit. In either case I enjoyed the visit.
