The hotel does not deserve four stars, not even two. The only good things about it are the views from some of the rooms and the restaurant staff, which are extremely nice and provide an excellent service.

They are a full service hotel. I am pretty sure they have a cot size bed for a child or other extra guests.

Garibaldi is the central train station of Naples. It's not at a walking distance from the hotel so you will need to take a bus from there to get to the hotel and the same on your way back. The bus ride is about 10 minutes (bus 151, you need to buy tickets before boarding) but beware of pickpockets especially during crowded hours.
There's no shuttle service from the hotel but there are cabs that you can hail right at the hotel's entrance. That's the most convenient option I reckon.
