The room itself is very small, and though it is 10 minute walk to Vatican City, it is located at quite a distance from most other sites, such as the Colosseum and the Pantheon.
My experience with this lodging owner was also unpleasant.
The hotel was difficult to find. It's not a hotel as such, more of an apartment converted into a few rooms. Very nice room though, fantastic shower and fully functioning air-con.

Ideal location near metro Station and Vatican city. Typical Italian suburb with superb caffes, mini market and risorante in range. Clean conformable rooms, free Wi-Fi with reasonable prices. Very helpful personnel. Next time in Rome definitely will be selected again.

For both of your questions the answer is YES. I have just checked a website and it is the B&B's mandatory charge. This certainly was not the case when I was staying there October 2015. Breakfast does consist of only tea, coffee, or juice with a croissant. Breakfast is in Cafe Doria which is situated around the corner from the B&B. I suggest you read my review for a true opinion for this B&B. The only good thing was its proximity to the Vatican. I hope you have a great stay at this B&B however if I knew what I know now, I would have certainly booked a different hotel/B&B.

Our transfer have a special price each way for up to 3 passengers and luggage, and it takes about 40 minutes from the airport to our B&B.The price will be the same even in case of delay of your flight.
If you book it you'll find a taxi driver inside the airport (just outside the luggage claim area) with a sign of your name waiting for you, and you have to pay directly to him.
If you are interested about the taxi service let me know your flight number, time of landing and mobile phone!
Otherwise, Train LEONARDO EXPRESS costs 14 euros per person and it leaves every 30 minutes from plat 1 in the airport. It takes about 45 minutes until Termini railway station and from there you have to take metro line A until Ottaviano-S.Pietro. After that, you must walk for about 400 meters until Via Bu Meliana 12 (our B&B).
Below directions to reach our B&B from metro stop ''Ottaviano-S.Pietro'' by walk:
Proceed west on Viale Giulio Cesare (walking in the opposite sense of driving direction)
Turn right onto Via Leone IV Continue straight on Via della Giuliana and after a big cross
Turn the second street left onto Via Bumeliana 12 Rome RM
