I am trying to find out the level of acceptance for transgender and gay people in Manuel Antonio and at the adults only resorts.
When the Manuel Antonio area first started to see the development of tourist infrastructure, It was known to be, and was marketed as, a Gay friendly destination.
Can't answer your other questions, sorry.
I have had gay clients stay in Manuel Antonio and enjoy it.
As for adults only properties, our stay at Tico Tico Villas was wonderful. Exceptionally kind hosts & well maintained small complex. Very reasonably priced too.
Link below is mostly advertising, but is a source of some local info nonetheless
Manuel Antonio is a very gay friendly destination in Costa. Rica.....for both tourists and Costa Ricans......it has been since the '80's......there is a large gay population (of locals and expats) that live in Manuel Antonio year round......and many businesses are goy owned/operated.......however one of the things that makes Manuel Antonio so special is that it has a very diverse and open mix of people......from sports fishermen, international tourists, artists, local families, surfers.......very friendly and welcoming......like most all Costa Ricans.

I agree with what has already been written.
This recent article - though not about the MA area - may provide some background and context.
