There are a lot of tourist-trap-white-table cloth restaurants with high prices and bad fare in Otranto; Balconcino d'Oriente is definitely not one of them. It's rather an honest and quality place with great food, reasonable prices and romantic but down to earth atmosphere.
If you appreciate a low key tasty food and some great rosato, this is the place to go to. When I was there, the waitresses were two older ladies that showed no mercy - they would boss around and make sure everything in the restaurant was as they wanted it; in an adorable yet professional way. It reminded me a little of the Seinfeld episode "the soup nazi".
I had some Orrechiette with veggies on ricotta as a Primo Piato (apparrently one of their house plates) and it was absolutely spectacular.
